LIST OF REQUIREMENTS ON CDIF WITHDRAWAL
Project A (FLGC/NFA-JICA FAP Payment)
1. Application for CDIF Withdrawal ;
2. FO’s Board Resolution “stating therein the specific project where the CDIF will be utilized, stamped “Verified Correct” on every page signed by the Provincial Investigator,”authorizing the NFA to deduct from the FO’s CDIF the whole amount of loan per amortization schedule provided in the MOA and Loan Agreement of the FLGC or the Deed of Sale with Reservation of Title of the NFA- JICA FAP;“In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required.”
3. Certification from NFA issued by the NFA Provincial Accountant that the FO has an outstanding PHF loan under the FLGC or NFA-JICA FAP, including the schedule of amortization, interest and penalties; and
4. Inspection report by the Provincial Investigator regarding the compliance of the FO to the terms provided in the MOA and Loan Agreement of the FLGC or the Deed of Sale with Reservation of Title of the NFA-JICA FAP.
5. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project B (Outstanding Loan Balance / Arrearages with NFA)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF will be utilized, stamped “Verified Correct” on every page signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required.
3. Certification issued by the NFA Provincial Accountant that the FO has past due obligations with the NFA including a schedule of amortization.
4. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project C (PHF Acquisiton, Repair, etc.)
1. Application for CDIF Withdrawal ;
2. FO’s BoardResolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every page and shall be signed by the Provincial Investigator. In case the withdrawal is more than P50,000.00, an Assembly Resolution shall be required.”
3. Certification issued by the NFA Provincial Accountant that the FO has no past due obligations with the NFA and that the amount of the FO’s accumulated CDIF has a sufficient balance to pay the equivalent amount of its FLGC/JICA two year amortization if the FO has an existing FLGC/JICA loan;
4. Specifications of the PHF to be acquired;
5. Type of repair/ service needed with the Scope of Repair, as submitted by the FO, approved by the Provincial Engineer or any technical personnel of the PO authorized by the Provincial Manager;
6. FO canvass of at least three dealers of PHF to be acquired or Price quotations of service charges for PHF repair;
7. Purchase Order with supplier’s certification of availability of after sales service and spare parts; and
8. Statement of Utilization of Previously Withdrawn CDIF supported by certificates/receipts or any proof certifying that the amount previously withdrawn was properly utilized in accordance with the purpose for withdrawal (e.g.
Official Receipt
of PHF’s dealer).
Project D (Rental of NFA - Owned PHF)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every pare and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required;
3. Appropriate rental rate certified correct by the Provincial Industry Services Officer (PISO);
4. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project E (Loans with lending institutions)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every pare and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required;
3. Certification issued by the NFA Provincial Accountant that the FO has no past due obligations with the NFA and that the amount of the FO’s accumulated CDIF has a sufficient balance to pay the equivalent amount of its FLGC/JICA two year amortization if the FO has an existing FLGC/JICA loan;
4. Certification from the bank or any lending institution that the FO has an outstanding PHF loan including the schedule of amortization and amount of outstanding loan balance;
5. Copy of the loan agreement between the FO and the lending institution;
6. Inspection report by the Provincial Investigator indicating that the said PHF is operational and in the possession of the FO; and
7. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project F (MTS Deposit)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every pare and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required. The resolution shall state the expected volume of stocks to be delivered to the NFA which shall be contained in MTS to be borrowed from NFA; and
3. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project G (NFA conducted training/s)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every pare and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required;
3. Cost of training and cost components prepared by the PISO, approved by the Regional Manager and confirmed by ISD; and
4. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project H (NFA licenses/registration)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution that states the license/ registration the FO shall obtain from the NFA. For payment of real estate tax or insurance premium for PHFobtained under FLGC program, the FO shall specify in its resolution that the FO authorizes the NFA to deduct from its CDIF the amount for real estate tax/ insurance premium. The resolution shall be stamped “Verified Correct” on every page and shall be signed by the Provincial Investigator.
3. Cost of License or Registration/ Tax Assessment/ Insurance Bill certified correct by the PISO; and
4. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project I (Purchase of NFA Rice, Fertilizer and Certified Seeds)
1. Application for CDIF withdrawal;
2. Copy of Assembly Resolution requesting NFA to utilize CDIF for payment in kind (PIK) or purchase of farm inputs.” The resolution shall be stamped “Verified Correct” on every page and shall be signed by the Provincial Investigator.
3. List of farmer beneficiaries with corresponding CDIF due to them.
4. Payment thru PIK shall be through voucher system payable to the FO which shall issue

5. Withdrawals of PIK shall be supported by an authority signed by farmer beneficiaries. Prioritize issuance of 5% and 15% broken IMR for PIK payment.
6. Payment of CDIF to purchase farm inputs shall be canvassed from at least three (3) registered Fertilizer and Pesticides Authority dealers. Payments shall be thru voucher system payable to suppliers of each FO supported with names of farmer beneficiaries and corresponding quantity.
7. Canvass from at least three (3) registered Fertilizer and Pesticides Authority dealers.
8. Representatives in the withdrawal of inputs shall have an authority signed by farmer- beneficiaries.
9. Certification issued by the NFA Provincial Accountant that the FO has no past due obligations with the NFA and that the amount of the FO’s accumulated CDIF has a sufficient balance to pay the equivalent amount of its FLGC/JICA two-year amortization if the FO has an existing FLGC/JICA loan;
10. List of Availees (names of farmer-members who will avail of the fertilizer/certified seeds; and
11. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project J (Purchase of Production Facilities like hand tractor, sprayers, reapers etc.)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall beutilized, stamped “Verified Correct” on every page and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required;
3. Certification issued by the NFA Provincial Accountant that the FO has no past due obligations with the NFA and that the amount of the FO’s accumulated CDIF has a sufficient balance to pay the equivalent amount of its FLGC/JICA two-year amortization if the FO has an existing FLGC/JICA loan;
4. Specifications of the production facilities to be acquired;
5. Canvass from at least three (3) suppliers of equipment to be acquired;
6. Purchase Order (PO). The PO should specifically state in the acceptance term that check payment shall only be made upon test run and acceptance of the facility by the FO and NFA; and
7. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
Project K (Payment for Production Loan and/or Other Loans, Obligations, Loan Arrearages with Government Institutions)
1. Application for CDIF Withdrawal;
2. FO’s Board Resolution stating therein the specific project where the CDIF shall be utilized, stamped “Verified Correct” on every page and shall be signed by the Provincial Investigator. In case the withdrawal is more than Php50,000.00, an Assembly Resolution shall be required;
3. Certification issued by the NFA Provincial Accountant that the FO has no past due obligations with the NFA and that the amount of the FO’s accumulated CDIF has a sufficient balance to pay the equivalent amount of its FLGC/JICA two-year amortization if the FO has an existing FLGC/JICA loan;
4. Certification from the government lending institution that the FO has an outstanding loan including the schedule of amortization and amount of outstanding balance;
5. Copy of the loan agreement between the FO and the lending institutions; and
6. Statement of Utilization of Previously Withdrawn CDIF (Exhibit 3) supported by
Certificates/Official Receipts
or any proof certifying that the amount previously withdrawn was utilized in accordance with the purpose for its withdrawal.
