E. Travel Accident Insurance
1. All NFA travelling officials and employees in the Central and Field Offices shall secure their travel accident insurance coverage not exceeding Two Hundred Thousand Pesos (P 200,000) from the Head Office of Government Service Insurance System (GSIS), Underwriting Department for the duration of their official travel to cover accident, loss of life or other allied risk.
2. A Central Office travelling employee shall furnish HRMD - Personnel Benefits Unit (PBU), a copy of his/her Travel Authority (TA) before 12:00 noon, not later than a day before his/her actual departure together with a transmittal memo signed by the employee and noted by the Head of Office of the travelling employee (Please see Exhibit 1.A - Letter Request for an Availment of Travel Accident Insurance Coverage for Central Office Personnel, of SOP No. HR-PAO8.D on Amendment/Addendum to the Revised SOP on Domestic Travel).
3. In the field offices, Travel Authority shall be submitted to the Administrative and General Services Section (AGS)/ RAOs/PAOs before 12:00 noon, not later than a day before his/her actual departure together with a transmittal memo signed by the Section Chief of the travelling employee (Please see Exhibit 1.B - Letter Request for an Availment of Travel Insurance Coverage for Field Office Personnel, of SOP No. HR-PA08.D on Amendment/Addendum to the Revised SOP on Domestic Travel).
4. The HRMD/RAOs/PAOs shall send the letter request for insurance coverage of travelling officials and employees signed by the Department Manager HRMD/Regional/Provincial Manager, to the Head Office of GSIS Underwriting Department together with a list of travelling NFA officials and employees requesting for insurance coverage (Pls. see Exhibit 2.A- Letter Request for Insurance Coverage for Central Office, of SOP No. HR-PA08.D on Amendment/Addendum to the Revised SOP on Domestic Travel and attached

- Letter Request for Insurance Coverage for Field Offices). The letter request shall be sent to GSIS thru fax every 10th day of the succeeding month. Upon receipt of the same, GSIS shall issue a Travel Accident Insurance Policy with the NFA.
5. A monthly billing for premiums for travel insurance coverage of employees who travelled for the month will be forwarded by GSIS to HRMD/AGS of the Regional/Provincial Offices on the following month. Upon receipt of the monthly billings, HRMD-PBU/AGS shall prepare the disbursement voucher for the monthly payment of premiums of those who travelled the previous month. The disbursement voucher shall then be forwarded to DAS/Finance Section for processing. In the field offices, remittance of premium shall be at the respective local GSIS offices.

Family Visits of Regional Managers, Assistant Regional Managers and
Provincial Managers
NFA Executives who are assigned to NFA regional/provincial offices located outside their places of residence except those who were reassigned as a consequence of disciplinary action, shall be allowed regular family visits on official time, and special travel for emergency purpose as necessary under the following conditions :
1. A Maximum of ten (10) family visits shall be allowed for the Regional Managers in a given year, eight (8) for Asst. Regional Managers and six (6) for Provincial Managers/OICs; each visit not to exceed three (3) days including travel time;
2. Only travel expenses shall be allowed for reimbursement; i.e. excluding per diem;
3. Availment of customary mode of transportation; use of air transport shall be resorted to, only when the point of destination cannot be reached by land and sea transportation.
The Regional Managers and Assistant Regional Managers/Provincial Managers/OICs shall notify the Deputy Administrator for Finance and Administration, the Assistant Administrator for Administration and the Regional Managers respectively, at least one (1) week prior to the intended travel.
