For the information, guidance and compliance of all concerned, hereunder is the amendment/ addendum to the Revised SOP on Domestic Travel which was approved May 10, 1999 and made effective May 17, 1999.

Travel Accident Insurance
1.
An NFA travelling employee based in central office shall secure a Travel Accident Insurance coverage not exceeding Two Hundred Thousand Pesos (P 200,000) from the GSIS, Underwriting Department for the duration of his/her official travel to cover accident, loss of life or other allied risk. In the field offices, insurance coverage for travelling officials and employees shall be obtained from the local GSIS office, General Insurance Division by the RAO/PAO.
2. A travelling employee shall secure a travel accident insurance coverage and shall furnish HRMD - Personnel Benefits Unit (PBU)/Administrative and General Services (AGS) a copy of his/her Travel Authority (TA) before 12:00 noon, not later than a day before his/her actual departure. This shall be submitted to HRMD/AGS with a transmittal memo signed by the employee and noted by the Head of Office (
for Central Office)
and the Section Chief of the travelling employee (for R.O./P.O.) (pls. see attached
Exhibits 1.A and 1.B).
3. The Personnel Benefits Unit of the Human Resource Management Department (HRMD) / RAOs/PAOs of the AGS in the field offices shall be responsible for securing travel accident insurance coverage for travelling officials and employees. The HRMD/AGS shall send a letter of request signed by the Department Manager HRMD/Regional/Provincial Manager, to the GSIS Underwriting Department (for central office)/ General Insurance Division (for local GSIS office) (please see attached
Exhibits 2.A and 2.B
) to formally secure travel accident insurance coverage together with a list of travelling NFA officials and employees. The letter request shall be sent to GSIS thru fax not later than a day before the expected date of departure of the travelling employees. Upon receipt of the same, GSIS shall issue a Travel Accident Insurance Policy with the NFA.
4. A monthly billing for premiums for travel accident insurance coverage of employees who travelled for the month will be forwarded by GSIS to HRMD/AGS every 2nd week of the following month. Upon receipt of the monthly billings, HRMD-PBU/AGS shall prepare the disbursement voucher for the monthly payment of premiums of those who travelled the previous month. The disbursement voucher shall then be forwarded to DAS/Finance Section for processing.
5. In case of accident, HRMD/AGS shall send a notice of the accident to GSIS Underwriting Department (for central office) / General Insurance Division (for field offices), within 24 hours upon occurrence or at a reasonable time giving the following information :
a) person/s involved in the accident;
b) nature/cause of accident;
c) date and place of occurence;
Documents required to facilitate insurance claim for accident are as follows:
a) Police Report
b) Travel Authority (TA)
c) Certificate of Employment
d) Medical Certificate/OR
The total insurance claims collectible for an accident shall be computed based on the insured value as computed by GSIS.
6. In case of loss of life, the beneficiary/ies shall claim the full amount of Two Hundred Thousand Pesos (P 200,000.00). The following necessary documents shall be submitted to HRMD - PBU/AGS in three (3) copies.
a) Letter of Manifest (travel thru sea and land)
b) Certificate of Employment
c) Marriage Contract (if married)
d) Death Certificate
e) Police Report
HRMD/AGS shall then forward the application for travel accident insurance claim to GSIS Underwriting Department (for central office) /General Insurance Division (for local GSIS office) for processing.
7. The HRMD/AGS shall be responsible for following up all insurance claims filed with the GSIS. All developments related to insurance claims shall be communicated in writing by HRMD/AGS to the claimant.

All rulings and/or issuances inconsistent herewith are hereby superseded.
