Educational needs of the employee and the immediate members of his/her family; vi. Other analogous cases as may be determined by the Civil Serice Commision. a. Letter Request addressed to the Department Manager of HRMD;
b. Police Report; and, 4) applicant must be a permanent employee. 7) applicant must not have any current foreign or local scholarship grant. 8) applicant must have fulfilled the service obligation for any previous study leave, scholarship/training contract.

7.9 Requests for study leave shall be supported by the following documents: E. PROLONGED LEAVE OF ABSENCE The respective heads of offices/departments shall be responsible for monitoring and ensuring that rules, policies and procedures on leave of absence and tardiness are strictly implemented and complied with especially those applicable to prolonged and unauthorized absences.
H. MAINTENANCE OF PERPETUAL LEAVE CARDS (PLC)
I. TRANSFER OF LEAVE CREDITS 3. The transfer of accumulated leave shall be claimed within one (1) year only from the employee’s transfer to the new agency. II OTHER PROVISIONS 1. Absence on a regular day for which suspension of work is announced. III. SANCTIONS IV. RESPONSIBILITIES
1. NFA Officials and Employees Ÿ Accomplishes and signs ALA in three (3) copies. Copy distribution of accomplished/approved ALA shall be as follows : Ÿ Attaches the required supporting documents to ALA.
Ÿ Forwards ALA to recommending official. 2. Recommending/Approving Official Ÿ Signs and approves filed ALA.
Ÿ Forwards/endorses ALA to PBU - HRMD/RAO/PAO for processing. Ÿ Retains copy for reference and file
Ÿ Updates perpetual leave card of employees at the end of the month - indicates leave earnings for the month and add to employees’ leave balance
- affixes initials on every entries made on leave card B. Reporting/Monitoring of Absences/UDT/Tardiness

1. RAO/PAO § The employee whose approved leave has expired has reported back to work;
§ The official/employee has been on authorized leave extension, or;
§ The official/employee has been recommended to be dropped from the roll due to AWOL.

The foregoing SOP supersedes SOP No. HR-PB37 approved April 24, 2002 and amendments issued thereto. All other rulings and/or issuances inconsistent herewith are also hereby superseded.

This SOP shall take effect on January 7, 2011..
