1.
Processing of Applications
1.1 All applicants (individual or entity) shall be required to indicate their permanent Tax Identification Number (TIN) in their applications for NFA permits, licenses, accreditation, and other documents (i.e., pre-qualification and bid documents) used for transacting business with the NFA.
1.2 The applicant shall also be required to present his TIN Card for identification purposes. For entity-applicants, the entity's TIN card as well as the TIN card of its authorized representative shall be presented. A photocopy of the TIN cards duly certified by the NFA personnel to which the cards were presented shall be kept on NFA’s file and made part of the person or entity's official records.
1.3 In case the applicant's TIN /TIN card has not yet been issued, his application for registration thereof (BIR form 1901/1902/1903) duly stamped by the BIR shall be presented.
However, this shall only be allowed until April, 2000 which is the expiration of the two-year period given to BIR pursuant to EO 98, within which all taxpayers should have been issued their TIN/TIN cards.
1.4 Upon issuance of his TIN and /or his TIN card, a photocopy of the same shall be immediately submitted to the NFA. The TIN card shall also be presented for authentication/verification purposes.

No application shall be acted upon unless the applicant's TIN is indicated therein

In case of pre-qualification and bid documents, non-indication of the TIN shall be a ground for the bidder’s automatic disqualification.

Revision of Forms
2.1 All NFA forms/documents affected by EO 98 shall be revised to incorporate the applicant’s TIN which shall be placed near the applicant’s name. However, in line with the government’s cost reduction program, revision of affected forms/documents shall be effected on the next requisition/reproduction.
2.2 The existing stock inventory of affected forms/documents shall be utilized until depleted. The provision for the applicant’s TIN shall be incorporated in the existing forms using a rubber stamp.

Coverage
E.O 98 shall cover only transactions between NFA and third parties (i.e. farmers/farmer organizations, retailers, millers, contractors/suppliers, bidders, etc.). For transactions between NFA and its employees (i.e. clearance from money and property accountabilities, service record, certificate of remuneration, etc.), the employee’s TIN need not be indicated.

Sanctions
Failure to comply with this guidelines shall be subject to appropriate sanctions provided for in the National Internal Revenue Code and other pertinent laws and regulations.
III. RESPONSIBILITIES
1. All heads of concerned departments shall ensure that:
1.1 Applications/pre-qualification/bid documents of persons or entities without valid TIN/TIN cards shall not be acted upon.
1.2 All NFA forms/documents affected by EO 98 are modified/revised to incorporate the TIN of the person or entity transacting with the NFA, e. g.
BRD Dept. Manager - application forms and licenses for retailing, wholesaling, milling, etc.
DMO Dept. Manager - Farmers’ Passbook, Master’s Passbook, Certification of Accreditation, etc. and applications therefor
GSD Dept. Manager/ - Pre-qualification and bid documents including LOs, POs,
PBACs/RPBACs and other forms of contract between a supplier and NFA
2. For affected accountable forms, GSD shall ensure that the provision for the applicant’s TIN is incorporated (application forms and permits, licenses, etc.) in its next requisition.
This guidelines shall take effect this
