EXHIBIT A

SCHOLARSHIP SERVICE CONTRACT

The Government of the Philippines through the __National Food Authority (NFA)__, represented by _______(NFA Administrator)_______, with principal office and place of business at Sugar Regulatory Administration (SRA) Building, North Avenue, Diliman, Quezon City, herein referred to as the GRANTOR; ______(name of grantee)____, of legal age, Filipino, Single/Married, presently residing at ______________________________herein referred to as the GRANTEE; ________(relative)_______________, ______(officemate1)_________ and ________(officemate2)__________, of legal age, Filipino, Single/Married, presently residing at ____________________________, ________________________, and _________________________ herein referred to as the GUARANTORS.

WITNESSETH :

That pursuant to the provision of E.O. 129 as amended and in consideration of the grant and acceptance by the Grantee of a scholarship to undergo on official time, the grantee hereby agrees to fulfill the following terms and conditions:

1. That the grantee shall bind himself/herself to the terms and conditions and keep up with the necessary standards of accomplishment of the local/foreign scholarship program of NFA as specified in the SOP on the Grant of Local and Foreign Scholarships;

2. That the grantee shall conduct himself/herself in such manner as not to bring disgrace or dishonor to him/her and/or the National Food Authority;

3. That the grantee shall continue to receive his/her salaries and allowances and other emoluments and/or adjustments; if any, during the period of his/her approved scholarship;

4. That within thirty (30) working days after the end of every term for the duration of the scholarship, the Grantee shall submit to the Human Resource Management Department (HRMD), copy furnished the Representative for Administrative Matter (RAM) / Regional Administrative Officer (RAO) / Provincial Administrative Officer (PAO) a certified true copy of his/her semestral/ trimestral/quadmestral grades;

5. That in order to maintain the scholarship grant, the grantee should have no failing grade in any subject and should have a general weighted average of 2.0, or its equivalent, for Graduate or Post-graduate.

6. That in case the grantee fail to submit the requirements mentioned in item 4, he/she shall be subject to the withholding of his/her salaries, allowances and other benefits until such time that he/she has submitted said requirements;

7. That within thirty (30) working days after graduation, the Grantee shall submit to HRMD, copy furnished the RAM/RAO/PAO a certified true copy of his/her Transcript of Records, Certificate of Graduation or Diploma and the Re-Entry Action Planning (REAP);

8. In case the grantee fail to submit the requirements mentioned in items 4 and 7, he/she shall be disqualified to attend future trainings, conferences, conventions, etc. in accordance with the provisions of the SOP on the Grant of Local and Foreign Scholarship;

9. Upon completion of the grantee’s scholarship, he/she shall return and secure a Certificate of Assumption to Duty duly signed by the Head of Office and submit to HRMD for notation and RAM/RAO/PAO copy furnished ASD;

10. That the grantee shall serve the NFA for the period of ______ years corresponding to the ______ years of scholarship grant and apply the learning he/she gained from it; and in case of his/her failure to fulfill his/her service obligation due to resignation, retirement, transfer to other government agency or separation from the NFA service through his/her own fault or other causes within his/her control, he/she shall refund the proportionate amount of salary, allowances and other benefits received during the period of scholarship based on the formula provided in the II.I.2 of the SOP;

11. In case of the grantee’s failure to finish his/her local/foreign scholarship due to illness, he/she will submit to the HRMD/RAO/PAO a Medical Certificate duly authenticated by the concerned school/sponsoring agency (if local) or the Philippine Embassy in host country (if foreign), subject to evaluation of HRMD and any government physician;

12. In case of the grantee’s failure to pursue the scholarship or should it be cut short due to his/her own fault or neglect, he/she shall immediately inform HRMD in writing, through his/her Head of Office;

13. That in case of the grantee’s failure to fulfill any or all of the foregoing terms and conditions due to his/her own fault or willful act that will cause his/her disqualification, cancellation of his/her scholarship and his/her recall, such as but not limited to item Nos. 4, 7,10 and 11, he/she shall be obligated to refund to NFA in full, such sum of money that may have been defrayed by the Philippine Government and/or sponsor, the transportation and other expenses incidental to his/her scholarship grant, including salaries, allowances and other benefits received while on scholarship;

14. That the grantee shall not be allowed to attend another scholarship program, nor go on vacation leave of absence with or without pay for a continuous period of more than two (2) months, until his/her existing Service Contract is fully served, unless he/she pay the monetary value of the unserved portion of his/her service obligation using the computation provided in the SOP;

15. That the grantee shall authorize the Accounting Services Department (ASD) to withhold his/her salary as specified in item number 6, 10, and 13 and to deduct such amount; and

16. In case the Grantee fail to pay the monetary value of his/her obligation, any unpaid amount shall be borne by his/her guarantor.

SWORN STATEMENT OF THE GUARANTORS
