11. In case of the grantee’s failure to finish his/her local/foreign scholarship due to illness, he/she will submit to the HRMD/RAO/PAO a Medical Certificate duly authenticated by the concerned school/sponsoring agency (if local) or the Philippine Embassy in host country (if foreign), subject to evaluation of HRMD and any government physician;
12. In case of the grantee’s failure to pursue the scholarship or should it be cut short due to his/her own fault or neglect, he/she shall immediately inform HRMD in writing, through his/her Head of Office;
13. That in case of the grantee’s failure to fulfill any or all of the foregoing terms and conditions due to his/her own fault or willful act that will cause his/her disqualification, cancellation of his/her scholarship and his/her recall, such as but not limited to item Nos. 4, 7,10 and 11, he/she shall be obligated to refund to NFA in full, such sum of money that may have been defrayed by the Philippine Government and/or sponsor, the transportation and other expenses incidental to his/her scholarship grant, including salaries, allowances and other benefits received while on scholarship;
14. That the grantee shall not be allowed to attend another scholarship program, nor go on vacation leave of absence with or without pay for a continuous period of more than two (2) months, until his/her existing Service Contract is fully served, unless he/she pay the monetary value of the unserved portion of his/her service obligation using the computation provided in the SOP;
15. That the grantee shall authorize the Accounting Services Department (ASD) to withhold his/her salary as specified in item number 6, 10, and 13 and to deduct such amount; and
16. In case the Grantee fail to pay the monetary value of his/her obligation, any unpaid amount shall be borne by his/her guarantor.
