GUIDELINES IN ACCOMPLISHING THE WAREHOUSE INFORMATION SLIP
1. The Provincial Office shall reproduce the guideline for the Warehouse Supervisor's/Accountable Officer's reference and guideline.
2. The Provincial Operations Officer shall be responsible in providing the warehouse supervisor data or information available at the provincial office. Aside from the basic items, the following shall therefore be entered in the Warehouse Information Slips before disseminating the slips to warehouse supervisors or accountable officers concerned.

3. The POO may coordinate with the accountant the contracted cost of construction or other items requiring data from the accounting unit.
4. The Warehouse Supervisor or accountable officer assigned to the waehouse shall accomplish the following items: (Refer to Illustration).
A warehouse information slip shall always be accomplished by a warehouse layout. One slip shall be accomplished for a duplex/triplex warehouse; while in the case of a warehousewith a main and an annex, two (2) separate slips; shall be accomplished; i.e. one slip for the main and one (1) slip for the annex. Each silo shall also have its own warehouse slip.
4.1 Measurement of Bodega (L,W,H (h1,h2), radius if silo.
4.2 Mill area utilized (in square meters)
4.3 Property Area utilized (in square meters)
4.4. Non-Grains Area Occupied (in square meters)
4.5 Actual Grains Capacity (in square meters)
4.6 Grains Stock Inventory (as of date of inventory, YY-MM-DD; i.e. every end of the month)
4.6.1 Actual Grains - Inside pile per stock condition
4.6.2 Actual Grains - Outside pile per stock condition
4.6.3 Grains Total (Inside and outside pile)
5. For those warehouses that are not yet entered or existing in the Warehouse Library maintained at MSD; a Warehouse Information Slip shall also be accomplished with all the information required, per Exhibit A. For the warehouse code item of those warehouse/silos with no assigned waehouse code yet, it shall be left blank. C.O. - DGM will assign the warehouse code and properly notify the province.
6. The Warehouse Supervisor/Accountable Officer shall certify the validity of information/data entered in the Warehouse Information Slips and forward to the POO concerned.
7. The POO shall validate the required entries and sign the "Noted By" portion of the accomplished slips.
8. All Warehouse Information Slips, per warehouse shall be submitted thru the Provincial Manager with an attached endorsement memo, with a listing of all warehouses who had accomplished the Warehouse Information Slips.
9. All Warehouse Information Slips shall be forwarded to DGM, Logistics and Processing Division.
10. MSD shall update/create the warehouse library or masterfile based on the properly accomplished slips. This serves as input or reference particularly on warehousing volume, utilization and capability reports per warehouse, province, regional and national summaries.
