We are hereby reiterating the first demand letter issued to you dated ______ requiring you to submit a copy of your Transcript of Records, Certificate of Graduation or Diploma and the Re-entry Action Planning (REAP) immediately relative to the scholarship granted to you by the National Food Authority (title of scholarship and duration). Since you have not submitted the said requirements, please pay immediately to the NFA Cashier the amount of _________________________________ (P ____) representing the total amount of expenses incurred for your scholarship. Otherwise, said amount shall be automatically deducted from your next month’s salary and/or receivables from NFA until fully paid.
