A.
PRINCIPAL
1. Company profile/brochure containing the following information :
a. Company history
b. Product Lines
c. Present Officers
d. Bank References
2. Financial Statements for the past three years.
3. Articles of Incorporation or Articles of Partnership, as the case may be.
4. Constitution and By-Laws (for corporation only).
5. Authenticated copy of Board Resolution appointing the agent in the Philippines to submit offers, negotiate and sign contracts and other documents in behalf of Principal.
If the Principal is a partnership, authenticated copy of the certificate appointing the agent in the Philippines.
6. Certificate from the local embassy/consulate of the principal duly signed and with embassy/consulate seal:
a. Business Address
b. Commercial Registration
c. Capitalization
d. Authorized Signatories
e. Lines of Business
7. Proof of previous performances as trader/supplier of the specific commodities intended to be supplied to the NFA as supported by a copy of a Bill of Lading and Certificate of Origin. Said supporting document should cover one boat load of at least 10,000 MT per commodity and should be dated not more than thre years from date of application counting backwards.
8. Certificate of membership from any grains association duly recognized by their government that principal is an international grains trader.
B.
AGENT
1. NFA license as indentor
2. In case of corporate agents, Board Resolution authorizing the officer to submit offers, negotiate and sign documents on behalf of the corporate agent.

Accreditation is on a per commodity coverage. The commodity/ies which you wish to supply should be indicated in your letter.
