ANNEX A

1. Warehouse Code (WHC) - A maximum of seven (7) characters where

- the first four (4) digits represent the region/province code;
- the next two (2) digits represent the warehouse number; and

- the last character differentiates the main warehouse from the Annex.

- The warehouse number ranges from 01-99 and corresponds to the different type/status of warehouses as follows:

01 - 19 : NFA-GID
20 - 39 : NFA-owned (old type)/Silo
40 - 99 : Leased/storage/free rental

2. Warehouse Name (WHN) Name assigned to a specific warehouse.

3. Warehouse Address (WHA) The place where a warehouse is physically located.

4. Distance from provincial office (DPO) The kilometric distance of the warehouse from the provincial office following the shortest route available.

5. Measurement of bodega :

1. Length (DGT) The longest horizontal side distance of the warehouse in meters.

2. Width (WDT) The shortest horizontal side distance of the warehouse in meters.

3. Height

3.1 Height 1 (HG1) The vertical distance from the floor of the warehouse to the top of the wall, at the slope of the roof. (optimum height).

3.2 Height 2 (HG2) The vertical distance from the floor of the warehouse to the apx of the roof (maximum height).

4. Radius (if silo) (RDS) One half of the silo diameter.

6. Type/Status of Contract (TCT) Classification/Status of each warehouse whether it is NFA-GID, NFA-owned (old type)/silo, leased, for storage or free rental. Codes used are as follows:

1. NFA-GID
2. NFA-owned (old type)
3. Silo
4. Leased
5. Storage
6. Free rental

7. Start Date of Contract (SDC) This is applicable to leased warehouses only; the date when the contract commenced.

8. Due Date of Contract (DDC) This is applicable to leased warehouses only; the termination date of the contract.

9. Name of Lessor (NOL) The name of the owner of leased warehouses.

10. Address of Lessor (AOL)

11. Monthly Rental (MTR) Rental cost/month of leased warehouse.

12. Rental/Sq. M (RSQ) Rental cost/month of leased warehouse divided by its floor area.

13. Acquisition Date (AQD) For GID-constructed warehouse, the completion date of construction for purchased warehouses, the date of acquisition.

14. Construction Date (COD) For NFA-constructed warehouses, the actual date of start of construction

15. Percent completion (PCC) Applicable to NFA-owned GID-constructed warehouses

16. Construction Cost (COC) Total cost of construction for NFA-owned-GID constructed warehouses

17. Name of Contractor (NOC) Contracting party for NFA-owned GID constructed warehouses

18. Mill area utilized (In square meters) (MAS) Refers to the area that is occupied by a permanently installed ricemill and its accessories

19. By-products area Utilized (BAS) Refers to the area that is occupied by the by-products of palay

20. Property Area Utilized (In square meters) Area that is used as storage for movable (PAS) properties (junk or operational), empty sacks and various post harvest facilities

21. Non-grains Area Occupied (In square Meters) Area in the warehouse which is being intended or (NAS) occupied as storage for Kadiwa commodities

22. GSCO Name (warehouse supervisor) Name of assigned and accountable grains (GSC) officer/warehouse supervisor
