RE-ENTRY ACTION PLANNING (REAP)
(For Both LOCAL AND FOREIGN SCHOLARSHIP)

This form is provided to ensure that all the learning/insights gained by the grantee shall be significantly applied to
his/her respective workplace/work unit. It is a proactive process that facilitates discussions and agreements between
the grantee and his/her supervisor on the focus of the strategy of the REAP.

The Re-Entry Action Planning promotes commitment building from both the employee and the supervisor to address the identified development concern of the NFA.
