DEED OF UNDERTAKING


I, ______________________________________ of legal age, Filipino
married/single and a resident of __________________________________

after having duly sworn to in accordance with law, do hereby depose and say:

1. That I applied and was duly accredited as an operator of
e-TRADE Sari-Sari Store (ESSS) upon paying a cash deposit
equivalent to P_____________________________;

2. That I am aware that the e-TRADE Sari-Sari Store aims to
ensure a stable supply of basic food commodities that are
affordable, available and accessible to the low-income
sector of the populace;

3. That I am also aware that being an accredited operator of
e-TRADE Sari-Sari Store is more of a public service, rather
than just a profit-making venture;

4. That I shall sell e-TRADE Commodities (EC) only to end-
consumers at prices not higher than the NFA-prescribed
price;

5. That I shall segregate and properly mark with "ESSS" all
the goods intended for sale under this program, and that
they shall be easily distinguishable from other goods that I
may sell in my store;

6. That I shall not sell or offer to sell commodities that are of
the same brand as the ECs;

7. That I shall conspicuously display at my store the prescribed
ESSS signboard and price bulletin board;

8. That I shall display the prescribed price tags at all times;

9. That I shall allow duly authorized NFA representatives to
inspect my store and storage room;

10. That I shall obey lawful instructions of NFA and other orders,
instructions and laws promulgated by duly constituted authorities;

11. That I shall return the ESSS signboard to the NFA should I
cease to participate in the program or NFA revokes my
accreditation;

12. That I understand that any violation of this Deed of Undertaking
entitles the NFA to suspend or even cancel my accreditation.
