DEMAND FOR PAYMENT OF STOCK/MTS SHORTAGES


From a copy of a Demand Letter we received from (name of COA Auditor),
you have been found to have incurred stock/MTS shortages, in the equivalent amount of

Demand is thus made for you to pay under the terms and conditions stated in SOP No. ______ issued on ______________, 1997 within TEN (10) working days from receipt of this letter.
